The You Thrive “House to Home” program is a first-time homeownership program that seeks applicants with a demonstrated need for housing and the financial ability to own a home, even if they would not qualify for a traditional mortgage.

Applications for homeownership are only accepted when homes are in process. Applications are not accepted on a rolling, ongoing basis; rather, applications are accepted in cycles that align with the progress of the homes in the process.

All applications for services must be filled out completely and include the required documents in order to be processed. Incomplete applications will be denied. Please read this important information before submitting an application for assistance.
Documents needed when applying for First-time homeownership are listed below:
• Copy of Social Security Card(s) showing the Social Security numbers) of the applicants
• Birth certificate or adoption papers for all children living in the household
• Divorce Decree/Marriage Certificate
• Record of alimony payments
• Court Orders for child support and child custody
• Verification of child support payments or resolution of status/printout of last six (6)
months trust
• One (1) most recent month of paystubs -30 days of all wage earners above 18 years of
age
• Last two (2) years SIGNED Federal Income Tax Returns with W-2s or 1099
• Award Letter and most recent 1099 for Social Security, SSI, VA Benefits, Retirement
Benefits, etc.
• Copies of your last two (2) months of bank statements (ALL pages, checking and
savings)
• DD 214 if Veteran or surviving spouse of Veteran
• Verification of rental history copy of the lease and/or name, address, and phone number for
• Must include government-issued ID: driver’s license, state ID, passport, green card.
